From: Premier Agents Network, Inc. (“PAN”), BEST Mortgage Lending Services LLC (“BEST Mortgage”) and BEST Insurance Agency, LLC (“BEST Insurance”)

This is to give you notice that PAN has a business relationship with BEST Mortgage, BEST Insurance and BEST Title LLC (“BEST Title”). PAN’s owner and agents indirectly own 50% of BEST Mortgage, 50% of BEST Insurance and 100% of BEST Title.


This also is to give you notice that BEST Mortgage and BEST Insurance have a relationship with each other and with PAN and BEST Title. The owners of 50% of BEST Mortgage and 50% of BEST Insurance indirectly own 100% of PAN and 100% of BEST Title. 


Because of these relationships, this referral may provide PAN, its owners and agents, and BEST Mortgage, BEST Insurance and their owners a financial or other benefit.

 

Set forth below is the estimated charge or range of charges for the settlement services listed. You are NOT required to use PAN, BEST Mortgage, BEST Insurance, or BEST Title as a condition for settlement of your loan on or purchase, sale, or refinance of a property. THERE ARE FREQUENTLY OTHER SETTLEMENT SERVICE PROVIDERS AVAILABLE WITH SIMILAR SERVICES. YOU ARE FREE TO SHOP AROUND TO DETERMINE THAT YOU ARE RECEIVING THE BEST SERVICES AND THE BEST RATE FOR THESE SERVICES.

  Flood Insurance $400-$12,000 per year depending on property value
BEST Title Title - Commitment Fee $15
  Title – Closing Protection Letter $87
  Title – ALTA Endorsement 8.1 $21
  Title – ALTA Endorsement 9 $21
  Title – Owners’ Title Insurance Policy $1.59-$3.05 per $1,000 of Coverage
  Title – Lender's Title Insurance Policy $1.59-$3.05 per $1,000 of Coverage

 

ACKNOWLEDGEMENT

I/We have read this disclosure form and understand that PAN, BEST Mortgage and BEST Insurance are referring me/us to purchase the above-described settlement service(s) and they and their owners and agents may receive a financial or other benefit as the result of this referral.
